Adenoma detection rate in high-risk patients

Madhusudhan R. Sanaka, MD, FASGE, FACG, from the Digestive Disease Institute, Cleveland Clinic, in Cleveland, Ohio, USA describes this Original Article, “Adenoma detection rate in high-risk patients differs from that in average-risk patients.”

The focus of this study was to determine and compare adenoma detection rates (ADRs) in high-risk versus average-risk individuals undergoing colonoscopy at our institution by multiple endoscopists. High-risk individuals included patients with personal history of polyps, family history of colon cancer, and family history of polyps.

This study was important in order to determine ADR in high-risk patients and to help develop guidelines for minimum threshold ADR in high-risk individuals, similar to average risk individuals. Our chart review study showed that high-risk individuals had higher ADR compared to average-risk individuals. This was seen in both men and women. Patients with personal history of polyps had the highest ADR among the high risk groups. Prospective studies are needed to study ADR in high-risk patients and guidelines are needed for threshold ADR in high-risk patients.
